Gestation is a term that describes the time between conception and birth during which a baby grows and develops in the mother’s uterus. Because so many important milestones take place during this period to indicate a pregnancy is progressing normally, it is common for women to track their pregnancies to ensure these milestones are hit at the appropriate time. Gestational age is the usual way for describing the age of the pregnancy, or how far along it is.
Usually expressed as a combination of weeks and days, gestational age counts from the first day of the mother’s last menstrual period to the present, so it technically includes about two weeks during which the woman was not pregnant. Gestational age helps guide prenatal care. In addition, it yields an expected due date and is the method most physicians use for dating a pregnancy.

Also see Appendix 8 — Guidelines for the Midtrimester Obstetric Scan This scan is available for all patients as part of routine obstetric management. Cochrane evidence indicates a reduction in induction for post-dates pregnancy, increased rates of termination for fetal malformation and increased diagnosis of multiple pregnancy. There is no current evidence that perinatal mortality rates are reduced by this investigation. It is not recommended as a screening test for aneuploidy.
Diagnostic limitations of the examination need to be discussed with the patient. Technical limitations which may reduce diagnostic accuracy at the time of the examination should be recorded e. A morphology scan may also be performed as a second opinion for confirmation or further investigation of pathology. The Ultrasound Examination The ultrasound examination should include: Fetal number; Fetal heart activity; Fetal biometry including BPD, HC, AC and FL; Anatomical survey see Appendix 8 – Guidelines for the mid-trimester obstetric scan ; Fetal movement; Amniotic fluid assessment; Cervical length; Placental site, distance of lower margin from internal os; and Maternal anatomy including uterus and adnexal masses.
Ultrasound estimation of gestation should be used in preference to menstrual dates if the discrepancy is 10 days and there is no previous dating scan or known conception from artificial reproductive technologies. Acknowledgement should be made that the dates have been changed; Fetal anatomical survey should be reported and commented on with respect to detection of malformations and quality of assessment. Examinations that are limited or suboptimal because of fetal position, maternal obesity or scars should be acknowledged.
A judgment should be made as to whether a repeat examination would improve diagnostic accuracy; Fetal abnormalities should be reported to the referring doctor on the same day to expedite management choice for the pregnant woman.
Routine ultrasound should not be offered or requested simply to confirm an ongoing early pregnancy in the absence of any clinical concerns, symptoms or specific indications. The purpose of the scan is to confirm viability, accurately establish gestational age, determine the number of viable fetuses, evaluate gross fetal anatomy and, if requested, assess the NT as part of the risk assessment for aneuploidy.
Document findings as per Early pregnancy ultrasound examination see above :. Once a live embryo is visible, the CRL should be used to calculate the due date. The MSD should not be included in this calculation.

My dating scan
First trimester scanning is useful to identify abnormalities in the early development of a pregnancy, including miscarriage and ectopic pregnancy, and provides the most accurate dating of a pregnancy. Technique First trimester scanning can be performed using either an abdominal approach or a vaginal approach. Abdominal scanning is performed with a full maternal bladder, provides a wider field of view, and provides the greatest depth of view.
Vaginal scanning is best performed with the bladder empty, gives a much greater resolution with greater crispness of fine detail. In circumstances where both approaches are readily available, the greater detail provided by transvaginal scans usually outweighs other considerations, and is preferred. The patient is scanned in the normal examination position dorsal lithotomy with her feet secure in stirrups and her perineum even with the end of the examination table.
These differences rarely effect gestational age dating by more than a day or two. ultrasound scan 1st Trimester Ultrasound Scan.
A first trimester early pregnancy and dating ultrasound is an examination you have during the first 13 weeks of pregnancy. Due to the size of the developing structures and embryo, it is uncommon to perform an ultrasound before 5 weeks gestation 5 weeks after the start of your last menstrual period. Ultrasound is not consistently able to confirm viability and fetal heart activity prior to 6 weeks gestation, so unless there is an urgent need to have an early pregnancy and dating scan earlier most of these ultrasounds are performed after 6 weeks and ideally at 8 weeks for dating purposes.
Urgent cases may still be performed earlier, please discuss this with your referring doctor. Your ultrasound may be performed transabdominally or transvaginally. Transvaginal ultrasound involves placing a thin transducer slightly thicker than a tampon into the vagina. The transvaginal ultrasound can offer extra detail to the examination as it allows the ultrasound probe to come into close proximity to the uterus womb.
Obstetric ultrasound, also known as prenatal or pregnancy ultrasound, uses high-frequency sound waves to produce images of a developing embryo or fetus. Your doctor will use information from obstetric ultrasounds to track pregnancy progress, gestational age and help predict delivery dates. During an obstetric ultrasound, you will be on a bed, usually on your back.
You may be asked to change position during your scan. Changing position helps move your organs and the fetus into a better position so the sonographer can capture high-quality images. If you have an Alberta Health Care card or valid health care card from out of province, there is no cost for an obstetric ultrasound except in Quebec.
called an ultrasound scan) and you’re able to take the pictures home. However, it’s To calculate your estimated due date (EDD) based on gestational age.

The information should not be construed as dictating an exclusive course of treatment or procedure to be followed. As soon as data from the last menstrual period, the first accurate ultrasound examination, or both are obtained, the gestational age and the estimated due date EDD should be determined, discussed with the patient, and documented clearly in the medical record.
Subsequent changes to the EDD should be reserved for rare circumstances, discussed with the patient, and documented clearly in the medical record. When determined from the methods outlined in this document for estimating the due date, gestational age at delivery represents the best obstetric estimate for the purpose of clinical care and should be recorded on the birth certificate. For the purposes of research and surveillance, the best obstetric estimate, rather than estimates based on the last menstrual period alone, should be used as the measure for gestational age.
For instance, the EDD for a pregnancy that resulted from in vitro fertilization should be assigned using the age of the embryo and the date of transfer. As soon as data from the last menstrual period LMP , the first accurate ultrasound examination, or both are obtained, the gestational age and the EDD should be determined, discussed with the patient, and documented clearly in the medical record.
For the purposes of research and surveillance, the best obstetric estimate, rather than estimates based on the LMP alone, should be used as the measure for gestational age. An accurately assigned EDD early in prenatal care is among the most important results of evaluation and history taking. This information is vital for timing of appropriate obstetric care; scheduling and interpretation of certain antepartum tests; determining the appropriateness of fetal growth; and designing interventions to prevent preterm births, postterm births, and related morbidities.

A dating scan is an ultrasound examination which is performed in order to establish the gestational age of the pregnancy. Most dating scans are done with a trans-abdominal transducer and a fullish bladder. If the pregnancy is very early the gestation sac and fetus will not be big enough to see, so the transvaginal approach will give better pictures. Dating scans are usually recommended if there is doubt about the validity of the last menstrual period. By 6 to 7 weeks gestation the fetus is clearly seen on trans-vaginal ultrasound and the heart beat can be seen at this early stage 90 to beats per minute under 6 to 7 weeks, then to beats per minute as the baby matures.
